A middle aged pt is brought to the ED because of the sudden onset of sharp chest pain and difficulty breathing a few hours ago. Temperature is 37c, pulse is 110/min and respirations are 36/min and shallow. He is slightly cyanotic. Exam shows deviation of the trachea to the right and subcutatneous crepitus on the left. What is most likely in this pt?
1. Dullness on percussion on the right
2. Hyperresonance on percussion on the left
3. Increased breath sounds on the left
4. Increased breath sound on the right
5. Increased vocal fremitus on the left
6. Whispered pectoriloquy on the right

Consistent with a Left-sided Pneumothorax?

Though i didn't know pneumothorax gave "SHARP" chest pains?

It's 2 nod

Yes I would go with 2 too... its a spontaneous pneumothorax... the subcutaneous crepitus may be subcutaneous emphysema...

  #8

yeah it is a case of spontaneous pneumothorax. So B is the answer
