An 80-year-old woman dies shortly after admission to the hospital because of a 3-day history of progressively increasing shortness of breath. She had a history of heart failure. Examination of the lungs during autopsy shows alveolar damage with distended lymphatics, congested alveolar capillaries, and pink-staining homogenous proteinaceous hyaline material in alveoli. Which of the following is the most likely cause of these findings?

A. Amyloidosis
B. Aspiration pneumonitis
C. Emphysema
D. Pneumonia
E. Pulmonary edema
F. Septic shock

  #2

b i guess!!

  #3

Pulmonary edemadue to pato findings (eosinetc) plus clinical presentation.

  #4

I guess F bcos sepsis & shock r the two most common causes of ARDS.

  #5

Sepsis is the most common cause of ARDS.......BUT......ARDS tends to affect the alveoloar-intersitial spaces.

Our patient has congested CAPILLARIES. So i'm going to have to guess Pulmonary edema due to our patient's history of Heart failure.

Anyone else?

Pulmonary edema,(both the path findings and pt's history)

  #7

How to explain pink-staining homogenous proteinaceous hyaline material in alveoli using pulmonary edema?

  #8

most common compl after AMI after 2-3 days - LVF - back flow - increase pr in pulm capillaries- leak into the interstitium - pulm edema

ans : pulm edema

  #9

yes I was, am still not sure whether its pulm.edema , just cos they mention pink proteinaceous substance. but i go for pulm.edema anyway

  #10

How @ pink staining protienaceous hyaline material in pulmonary oedema.Shouldn't we expect heart failure cells-hemosiderin laden macrophages?

  #11

A?

  #12

Nice question.. It's F

it says alveolar damage... which is ARDS, not APE

  #13

why not A?

  #14

"pink-staining homogenous proteinaceous hyaline material in alveoli" means exudations of protein NOT trasudation as it would be in pulm edema due to CHF. So Septic shock is the answer for me.

Septic shock -> ARDS

I amyloidosis you dont'have congeated lymphatics and capillaries, at least not that I know...
