If you are an IMG, it is better to have USCE. So do an externship or observership. But if you already have USCE or if you are an AMG, research experience looks great. With regards to LOR, it is suggested that you obtain it from program director, but LOR from any attending would do as long as it is a good letter.

california, Thanks a lot. Yeah, I am a IMG and just got my PhD here. I do not have any USCE yet. Seems do an externship or observership should make my application stronger.

there is a biiiiiiiiiig diff b/w observership and externship so try to do an EXTERNSHIP if possible, I've noticed from phoning programs that they're very particular about "on-hands" stuff. Gud Luck

The best and most accurate way to go would be to call various programs that your interested in and try to talk to the directors/administrators if they would let you do an externship with their program.
