a36 yr old man develops rapid mental status deterioration 2 days after sustaining a femoral fracture in a skiingple small accident.physiacl exam shows multiple smalll petechiae in the anteroir chest and abdomen.On the 3rd day,the man goes in coma and dies.On post martum exam ,there are numerous petechial hemorrhages in the corpus callosum and centrum semiovale.Which of the following is most likely diagnosis?

A.Diffuse axonal injury

B.Fat embolism

C.septic embolism

D.systemic thrombo embolism

E.watershed infarction

Fat embolism = Bergman Triad
  1. Petechiea in the upper part of the body
  2. Dyspnea
  3. Mental symptoms

any idea why it cant be diffuse axonal injury?it can also produce white matter patechea after trauma esp in the corpus callosum????is it bcz fat embolization is more commonly seen with fractures??

femoral fracture + petechiae--highly suggestive of fat embolism... i think it is the history of femoral fractue in the given case
