A 6 year old boy is brought to the Emergency Department by his parents. His temp is 103 F, and his blood pressure is 60/0. The boy has had severe headache and stiff neck for 12 hours. Cerebrospinal fluid cultures are positive for Haemophilus influenzae type b. Vaccination against which of the following would have prevented this infection?

A. Potent ADP ribosylating exotoxin
B. Peptidoglycan
C. Negatively charged capsular polysaccharide
D. Common core of LPS
E. IgA protease

The creation of the HIB vaccine in the mid 80's made the aforementioned scenario almost a thing of the past. Where we used to see a couple of CSF's a year positive with H. influenzae, this bug in CSF has virtually disappeared (thank goodness).

We do not see nearly the amount of this organism in other sites (mostly sputum specimens) that we used to either. It's hard finding a good slide to show students.

As far as treatment goes, in the olden days (70's and 80's) the patient used to automatically be put on ampicillin and chloramphenicol IV until sensitivities were in. Many H. influenzaes are now ampicillin resistant, so I do not know what the current protocol calls for CSF-wise.

Anyone know?
