The degree of anemia in a fetus with HDN is assessed currently with measuring the Peak Systolic Velocity of blood in Middle Cerebral Artery with U/S guidance.Although this has been reported in NEJM since 1997 , kaplan 2005-2006 Notes do not even comment on that.

Is this too "advanced" for USMLE step 2 exam ? It's also a non-invasive method comparing amniotic fluid bilirubin and PUBS.

any opinions?

was it a review or 'medical progress article' about HDN which mentioned it. if so, do keep it in mind. however UW duz not mention it either.

Dr Fischer addresed the q of beinh too recent' he said that its better to be as recent as possible, rather tan try to date urself back. also a recent exam experience hinted about some new vaccine on TV...as long as it wasnt a single study talking only about MCA stuff ( cud have been ruled out in the coming years or other techniques found superior) but a review/medical progress article , i d keep it in m y notes.

if you want to look it at NEJM the article is entitled as follows :
NONINVASIVE DIAGNOSIS BY DOPPLER ULTRASONOGRAPHY OF FETAL ANEMIA DUE TO MATERNAL RED-CELL ALLOIMMUNIZATION (2000)

last sentence of the article :

"In conclusion, measurements of the peak velocity of blood flow in the middle cerebral artery in fetuses[/left] [left]at risk for anemia due to maternal red-cell alloimmunization provide an accurate and noninvasive means of determining the degree of anemia"

Not only that. During my ob/gyn clerkship we have been tought that MCA-PSV is currently used to assess the degree of fetal anemia.I was surprised when i looked into Kaplan Notes and did not find it.[/left] [left]feel free to comment on that [/left]
