initially when there is vit-D def. due to renal failure, secondary hyperparathyroidism results due to low Ca+,which causes a dec. in PO4! however as renal failure progresses, tubulues become more and more unresponsive to PTH, and PO4 level normalizes or may even increase! remember tht! PO4 level is not very specific in renal failure!

yes, paradoxically po4 inreases with incrsd pth secr d/t incr bone resorbtion of po4 and inability of kidney to secrete em good qs ssrpk!
