I think most people would agree that it is easier to get a fellowship position on J1 than H1.Although exceptional candidates can get fellowship position on H1 as well.I recently had a talk with a former PD of endocrinilogy fellowship program over lunch, and according to him their criteria included medical school, usmle scores , performance during residency and in board exams and they preferred J1 over H1.But as we know the problem with J1 is to work in underserved community for 3 years and running the risk of staying unemployed for upto 6 months after residency if we fail to get COS from J1 to H1 within the cap for that year.

Almost agree with you. I think u need not be exceptional if you want a fellowship after h1. Most people who do residency on H1 finally end up doing fellowships. They apply for a hospitalist's job, work for 2 years, apply for a green card, keep doing research to strengthen their application, and then once they get a green card, they apply for a fellowship. But yes I agree that going for a fellowship right after residency, its hard on h1b. Basically you have to see your priorities in life.
