A

i also think its A. but incase of a tumor there shud be increased LH.right? why is LH decreased here/

  #5

EMPTY SELLA SYND THAT IS DUE TO PRESSURE NECROSIS

  #6

is empty sella syndrome due to pituitary tumor?

  #7

YES WHEN ITS SIZE INC --- PRESSURE --- BLOOD SUPPLY IS COMPROMISD----PITU NECROSIS

  #8

should be d, androgen decrease will lead to increase in LH.

  #9

i also think its D, but how can we explain it with pituitary tumor??

  #10

I think it may be a prolactinoma(tumor, decreased sex drive and bitemporal..)then prolactin levels suppress LH and FSH, therefore we have low LH and low testosterone and the rest of signs and symptoms,
so A.

  #11

yes, it is A

  #12

agree A
prolactin inhibits GnRH so FHS, LH, T are all decreased

yeah it may be prolactinoma but it may also be a non-functioning tumor which can very well be the reason for hypopituitarism (don't forget) !

empty sella syndrome is in most instances an incidental diagnosis, and pts' pituitary funstion is normal! in primary ESS, there is a defect in the diaphragma sellae which results in suprasellar subarachnoid invagination into the sella turcica and compresses the pituitary which on radiographs appear as empty space and hence the name! secondary causes include pituitary resection for any reason!
