I - S4 - late diastolic murmur caused by long - standing hypertension which produces ventric hypertrophy

this could be Aortic regurtation bc pulse pressure is wide almost 100mmhg diffeence and a diastolic sound heard in left lateral position is due to functional mitral stenosis caused by increased pressure in aorta which is pushing the anterior cusp of mitral valve(describe as Austin flint murmur in Text books),. i have never came across any book with this statement that S4 sound is heard in left lateral position.
