Pulse 96/min
Respirations 16/min
Blood pressure
Left arm 226/120 mm Hg
Right arm 218/118 mm Hg

With the patient in the left lateral decubitus position, a late diastolic sound is heard best with the bell at the apex. Which of the following is the most likely explanation for this auscultatory finding?

A) Aortic insufficiency
B) Aortic stenosis
C) Mitral insufficiency

I - S4 - late diastolic murmur caused by long - standing hypertension which produces ventric hypertrophy

this could be Aortic regurtation bc pulse pressure is wide almost 100mmhg diffeence
and a diastolic sound heard in left lateral position is due to functional mitral stenosis caused by increased pressure in aorta which is pushing the anterior cusp of mitral valve(describe as Austin flint murmur in Text books),. i have never came across any book with this statement that S4 sound is heard in left lateral position.
