I pick E. But don't know exactly why

Ventral horn refers to LMN.

Lateral column refers to UMN.

It's the only answer that gives bilateral motor deficits.

(Other answers are either one-sided lesions, or involve Pain & T deficits which our patient doesn't have)

Could this be Amyotrophic lateral sclerosis which shows a mixture of UMN & LMN signs......any comments?

That's correct, fox. It is typical presentation of amyotrophic lateral sclerosis.
