A 55-year-old woman notes a gradually increasing number of episodes of cramping abdominal pain and diarrhea that occur after ingestion of milk products. A decrease in which of the following best explains her condition? (A) Ability of enterocytes to transport calcium (B) Ability of Paneth cells to secrete lactase (C) Ability of Paneth cells to secrete lysozyme (D)Galactosidase activity at the cell surface of enterocytes (E) Sucrase activity within lysosomes of enterocytes

D. Im not sure if Paneth cells secrete lactase, from what i recall they don't.Paneth cells are large secretory cells with coarse granules found at the blind ends of the crypts of lieberkuhn. Kindly enlgihten me if they secrete lactase.thanks

Paneth cells do not secrete lactase. All disaccharidases are brush border enzymes, located in the microvilli of the duodenum (?). This is a classic presentation of lactose intolerance (except for flatulence), so it is a brush border enzyme deficiency, and D is the answer.

though none of the others other than D seem right, I thought brush border enzymes were disaccharidases.. galactosidase is monosacharidase.. I know fructokinase(monosacharidase) is present in the brush border, so probably galactosidase also is possible (though lactase would have been the easiest to identify the lactose intolerance).
