24 hours old neonate has a total bilirubinof 24 mg/dL. She was born at fullterm and weighed 6.6lbs. baby's apgars were 6 and 10 at 1' and 5' respectively. baby is active and otherwise well. what is the most appropriate therapy at this point of time?

hi alice, yeah, you are right. exchange transfusion is the treatment of choice for anybaby who has serum indirect bilirubin more than 20 mg/dL. exchange transfusion is needed even at lesser levels of bilirubin in preterm babies!

especially in the first week of life,which is blood brain barrier is not strong enough yet

what is kreatenism

i hope " kreatinism" referred to Cretinism - term used for congenital hypothyroidism. the word cretin is derived from French chretien, meaning little cherub or something. anyway the term was used in the past for cong hypothyroid infants who have puffy faces and mental retardation - somewhat like little cherubs! grin
