yeah zianab posting evryday helps a lot,,actually got busy wid few ceremonies yesterday,
have cousin`s marriage comng up this weekend,,so wuld have to do as much as possible this week ..i ve done 20 chaps of psychiatry frm notes,,completed 2 more lect..its already night here,,wuld try to finish two more lectures ,,

hi zainab,i am doing only kaplan,there's everything in kaplan,i am doing online UW,so i am supplementing kaplan with UW,trust me its more than enough,out of curiosity i checked few topics in CMDT,UW +kaplan covers most of CMDT.so i am sticking to these things,

covered psychiatry frm notes n lectures,( 1 lecture still left )...wuld do some questions before going off to sleep...doing ques frm NMS,,act don`t have any other book rite nw..
Plan to do biostats tomm..
zianab,,did u start psych? I too am not confident of med,,seems to have forgotten most of it...i think it ll be done well wen we do UW n memorise it..

act psychiatry was a nice change after med,,it took me abt 4-5 days,,i didn`t do the questions along ,, i have pickd them up today only..i think it won`t even take those many days as weekdays have less disturbances..u try it..
i ll do ques first chapter wise,,then do the comprehensive exam in the book...
zianab,,hws NMS ? act..i m finding ques too easy,,culd u suggest some other source??

u r right zianab...i too felt tht way wen i was doing ques ,,so i dropped it n am doing it frm blueprints psych nw..( the text one, it has a few ques at the end)...act don`t have other resorces rite nw...
act i just wanna finish this ` unending` frst read of the subjects so tht i start Q book,,so won`t be doing frm other ones...but i ll certainly do them wen i m on to my 2nd read n questions

me nw going to do ques,,cover the incomplete part..n will hopefully start wid my biostats..

Hiiiiiiiii Dr Pumpkin and Zainab
Missed u guys and our forum and studies all these days..The trip was gr8..but 12 hours drive was tiring..
The Neuroscience meet was huge..so huge that I couldn't pick up anything useful..But I'am happy that I could meet so many dedicated Neuro scientists and could know about their work.
So both of u have started psychiatry..good..and u both r finishing fast too..thats gr8.
I came the day before..yesterday I started pediatrics..finished New Born..I was tired and hence slow!!..I don't have a question source..will buy it soon..so for now it is Pediatrics theory..kaplan..
U guys keep doing well..good luck

i think blue prints is good fr reference ( thts wat people suggest on this forum ),,as fr the question sources i have pretest n nelson reveiew of peds,,so i d be doing it frm there (don`t knw abt their ratings though )...

Still doing Pedia..I think I will take some 5 days to finish it.
Why don't we post the amount of hours we seriously put in concentrated study each day..we may then be conscious as to how much time we waste unknowingly..
Zainab how much progress have u made?

well guys still Doing psyc.
I was reading in the forums about the type of qs they ask in exam and mostly they as:
what is the diagnosis
what is the intial next step u will do?
what is the next best step u will do?
what is the best diagnostic test?
what is the goldstandard test?
what is the first line of treatment?
what is the best treatment?
so I was thinking that we start from medicine and then we start from the first chapter and first topic and then each of us contribute to the above mentioned qs.I am doing qs nowadys and most of them ask this and I have no clue as in Kap they are not given in this manner and one of my friend who passed the exam in 90's she said that she made a complete diary of each disease in medicine and surgery (most imp )and this is the key to passing and I feel I am nowhere near to answer these kinds of qs.
So I AM STARTING FROM FIRST TOPIC IN MEDICINE AND WE WILL EACH DAY DO AT LEAST 2-3 TOPICS SO THAT ALL MEDICINE IS REVISED IN A fun manner.please before u post something be sure that u are posting the right answer and check it from cmdt ,harrison or e-medicine.ALSO WE WILL GO SEQUENCE WISE and go through every topic in kap book so anybody please donot post random topics.everybody is most welcome to post answers but please do follow the sequence in kap book.thankyou

This is a good idea..Let us get into the real thing rather than discussing the unimportant things..good work Zainab..
So lemme try to give the answers
A 1# Initial dx test ___ In case of GERD if u have proper history indicating the same u don't need an "initial dx test"..can go for the tx directly..
But if the question gives u only a chest pain as the symptom initial test would be to rule out the most dangerous Differential Dx and that would be an "EKG"
A 2# Most accurate test____it is Ambulatory esophageal PH monitoring(no doubts about this one) but this is unnecessary in most cases!!!Look out the indications for this.
A 3 and 4#Best initial therapy___Antacid/H2 Blockers/Metoclopramide(Mild to moderate ones) and for more serious ones it is PPIs.Summary is initial therapy is always medical not surgical.
A 5#I didn't get this..do u mean after the medical therapy..yes in 5% patients who doesn't respond can go for surgery(Nissen's fundoplication)
A 6#Indications of surgery....
a.Severe disease and patient not willing to take a lifelong medical Tx
b.resistant or intolerant to PPIs with erosive disease
c.otherwise healthy patient who has extra esophageal manifestation
A 7#Already answered
A 8#Side effects r tooo many..and would consume too much time..ok..can I leave it for someone else to answer

SOME OTHER TESTS USED FOR THE DIAGNOSIS ARE:
BERNSTEIN TEST ,barium swallow,chest xray.
EGD with biopsies should be performed if pateint has long standing symptoms to rule out barrets esophagus and adeno carcinoma
answer to q no 3:
First line of treatment"
is I guess is telling the patient of lefe style modification which are :
1---wt loss,elevate head of the bed while sleeping ,avoidance of nocturnal meals ,eat small meals ,smoking cessation,
then prescribing
2---ANTAACIDS ---- for patients with intermittent disease
3---H2 receptor antagonists (cimetidine,ranitidine) /metoclopromide
lastly in chronic pt
4--- PPI ( omeprazole,lansoprazole)
and refractory and severe disease do
5------Nissen fundoplication.

DRUGS USED FOR GERD AND THEIR SIDE EFFECTS:
please post anything I forget or say incorrect:
ANTACIDS:
CAN EFFECT ABSORPTION/BIOAVILABILITY OF OTHER DRUGS.
aluminium hydroxide------constipation and hypophosphatemia
mg hydroxide----diarrhea
ca carbonate----hypercalcemia,rebound acid increase
all can cause hypokalemia.
H2 BLOCKERS---
cimetidine,ranitidine,famotidine,nizatidine
cimetidine is potent inhibitor of p450.,anti-androgenic,and decreases renal excretion of creatinine.
PPI (OMEPRAZOLE,LANSOPRAZOLE)
didno find any woth remembering side effects.
