Thanku Mazinger..I know it was a silly question but it always helps to discuss.Which subject r u doing? Dr Pumpkin..its cricothyrodotomy which is done in a patient less than 12 years of age..And usually it is needed when there is massive facial trauma when intubation is not possible. By the way while u wasted 1 day I wasted 2 days out here..it was a long weekend.. Anyways we will finish off surgery fast,right?Paedia q sources r many..all half finished..Blue print,UCV,Pretest..hope I will finish all!! Zainab,where r u?Hope u r doing fine. Here is my take on Mazinger's NBME qs 1.A]Reassurance 2..A]Adjustment disorder with anxiety(though depression like symptom is also mentioned I think she has anxiety for sure) 3..B]Thyroid function test..this one got me confused..the child is surely lagging behind since he should be able to go to toilet himself and tell a whole story since he is 4..I think Thyroid dysFn is the most cmn cause of MR in boys..so why not rule out this.. 4..E]Threatened abortion..cervix is closed..fetus alive but HR is increased..and the product of conception is surely inside the uterus. good luck npas

npas: lets see on the answers 1st agree 2nd agree 3rd I think the answer is DNA testing, my diagnosis is a fragile X syndrome (X linked dominant), since its a familial thing you could check that the sister is also affected, but in a lesser degree. Enlarged testicles is the giveaway... 4th I think its a placenta previa, dont ask me why.. but I just know , the patient has not had any cramping which is compatible with painless bright red bleeding (I havent studied ob yet so I could be awfully wrong)..

Hey guys..I think Placenta previa is a good option in a late 2nd or third trimester bleeding..this lady is in her 12th wk..morover threatened abortions may sometimes present with only bleeding..cramping may develop after hours or days..please check this out and do let me know. 3rd one..I was wrong..I agree with u completely Mazinger. npas

i was thinking abt the placenta prev. question.... the reason for bleeding in placental previa is d/t rupturing of the blood vessels when the lower segment is streched...wich happens in the last trimestr... since in this cs, the uterus is just 12 wks gestational size,,,n there is no streching...so we culd think it as a rejection facor for PP n go for threatened abortion,,also as rightly said by npas,,it may present widout bleeding initially... wat do u guys think ??

ACE inhibitors are medications that inhibit the activity of the enzyme, which decreases the production of angiotensin II. As a result the blood vessels enlarge or dilate, and the blood pressure is reduced. This lower blood pressure makes it easier for the heart to pump blood and can improve the function of a failing heart. In addition, the progression of kidney disease due to high blood pressure and diabetes is slowed. ACE inhibitors are used for controlling 1-blood pressure 2-treating heart failure. 3-preventing kidney damage in people with hypertension or diabetes. 4-ACEI reduce mortality in patient after MI. 5-in patients with heart failure with LV dysfunction and ejection fraction less than 40%. ACE inhibitors usually are not prescribed for pregnant patients because they may cause Birth DEfects PT with severe kidney problems and people who have had a severe reaction to ACE inhibitors probably should avoid them. The most common side effects are cough,increase blood k levels, abnormal taste (metallic or salty taste), and rash. ACE inhibitors have interactions with other drugs. they may increase blood levels of potassium, the use of potassium supplements, k salt substitutes or other drugs that increase the body's potassium may result in excessive blood potassium levels. ACE inhibitors also may increase the blood concentration of lithium and lead to an increase in side effects from lithium.

DEAR MAZINGER, Well,you don't have to tell that why you didnot choose the right options.Each of us tell wrong answers and this forum is to discuss things not to judge each other,There are so many things I don't know probably lot more than you so keep on posting .And we will try to cover as much as we can.so there is an nbme cardiology qs left which nobody answered can you please answer that.I am not sure about that answer. also please npas,mazinger and dr.pumkin can you please posting cardiology qs so that we can go further with nbme and more cardiology qs. I know everybody hates cardiology and has the most trickiest qs at least for me so guys please answer atleast 2-3 qs each of you so we can go forward and thankyou all for posting answers. and dr.pumkin,I hope you feel better soon.I am doing surgery hope to get rid of it soon. You are right I dont have to... I was just making fun of myself, not everything is about medical knowledge I also like to laugh... My last post made me laugh a lot lol Did anybody laugh? at least a little??
