In Kaplan note, aspirin poison can cause hyperglycemia or hypoglycemia. What's the mechanism of aspirin-induced hyperglycemia?

Anybody knows?

hmm im just guessing but cud it be related to sympathetic activation..or the metabolic abnormalities produced?

Pediatr Dev Pathol. 2003 Jul-Aug;6(4):342-7.

A 5-year-old girl with a mild upper airways infection was admitted to the hospital because of sudden vomiting and drowsiness that evolved to stupor; she was dehydrated, hypotensive, and tachypneic; laboratory tests revealed noncompensated lactic acidosis. She also had hypoglycemia followed by hyperglycemia, and progressive bradycardia leading to reversible cardiac arrest. At autopsy, the most striking finding was multiple foci of coagulative necrosis involving the entire thickness of the myocardium with scant inflammatory infiltrate composed mainly of macrophages and polymorphonuclear leukocytes.

Authors explain:
metabolic acidosis leading to uncoupling of oxidative phosphorylation, inhibition of tricarboxylic acid, stimulation of lipid metabolism, and initial hypoglycemia due to uncoupling of oxidative phosphorylation evolving to hyperglycemia due to increased glyconeogenesis and stupor [9,23].

study_ing, thank you. It may be due to hypoglycemia and hypotension-induced activation of sympathetic nerve system.

Hyperglycemia
• Stimulation of adrenal medulla
• Stimulation of adrenal cortex
• Increased glucose-6-phosphatase activity

Hypoglycemia
• Increased glycolysis
• Impaired gluconeogenesis from non-carbohydrate precursors
