the most important factor to determine in a given case where u suspect bowel obstruction is to determine whether its simple or strangulated...this will make u choose btw the opion

A) n/g tube, NPO etc etc

vs

B laporotomy

( remember laxatives are always the wrong answer here)

so what wud one look for to decide simple vs compliccated?

signs like shock, fever, leucocytosis make you suspect a strangulation. pain is also likely to be continuos rather tahn colicky. as condition progresses to bowel infarction and gangrene, the cliniacl condition will worsen.
