differential:

electrolite problems
dehydration
high fever (depends)
epilepsy
trauma (recent) or old
malinguering
others...

the question will always suggest you what part of the differetial they want, but be careful with age, sex, treatment...etc and other factors messing with the patientīs health status.

they are different in a subtle form.

convulsion = seizure = a paroxysmal event
epilepsy is a condition of recurrent seizures due to a chronic underlying process.
