A 52 y.o. female come into your office for a regular consultation, she is asymptomatic without complains background is not relevant! P/E BP: 250/120 HR:85 bpm R: 13 rpm no focal neurologic deficits the rest of P/E is not relevant. What´s your next step? a) Start Sublingual captopril and refer to an ER b) Start sodium nitroprusside c) Send home and recheck on 2 weeks d) Call the ambulance because she´s on a Hypertensive Emergency. e) Send her to the hospital with no medication for re-evaluation.

I practiced medicine for 6 years and in my office option 1 is prefered.

she is asymptomatic, but BP is worrying...hmmm the answer must be (a) or (b). this is a hypertensive URGENCY, since there seems to be no organ damage...but option (a) is more prudent when pointing to send her to the hospital ER on captopril. that´s why I choose (a). but B is a good option also. my answer is (a)

B is practiced in hospital, not in your office.

a is answer-becouse it is faster and handy.It is like when you have AIM.First step is always Aspirin.Board likes this questions. b-is more complicate and takes time c-is obviously not an answer d-you probably won't call an ambulance without giving her anything and just waiting for the car? e-again you must do something!

250/120 is not an emergency situation and need prompt attempt to normalize the BP so I think B seem to be correct.
