i know this sounds like very basic thing but i could not understand why the LVEDV (left ventricular end diastolic volume) is decreased in septoc shock.

goljan rapid review says its bcoz of neutrophil emigration from pul.capillaries in to alveoli

i just dont get this point can any one plz explain this to me.

Well in septic shock there is a multiple organ failure,hypotension {refractive}
there is a massive vasodilatation beause the body is fighting back but it can't there is a systemc infection

  #5

septic shock = vasodilatin, due to endotoxin induced releas of NO, PGI2 & (alterntive complement) C3a & C5a(anaphylotoxins)..............

this means,vasodilation--------------> inrease TPR/PVR(resistance)-------------------->increased CO

and the book says decreased LVEDV ?????? dont get this , i mean with an increase in CO how can u have a decrease in LVEDV

Yes Vasodilatation

  #7

Septic shock has two phases

Hypedynamic phase --- in which CO is high and CVP is low

Hypodynamic phase ---- in Which CO is low and CVP is high

Initial there is decrease in venous return in the presence of excessive CO (due to increased contractility) and peripheral vasodilation .Venodilation causes decrease in venous return which in turn causes decrease in LVEDV. But this is in Hypedynamic state

In the later phase LVEDV should increase

Best of luck
