For the past 3 years, a 48-year old ma has been taking 1000 mg of acetaminophen every 4 to 6 hours for back pain. Each day, he has also been consuming at least eight bottles of beer. One morning after partying he takes 2000 mg of acetaminophen as soon as he gets out of bed . He experiences severe abdominal pain. He goes to the energency and is found to have ALT of 120 U/L. Which of the following P450 izozymes was liely induced by ethanol and responsible for the formation of the toxic metabolite of acetoaminophen? A. CYP2C9 B. CYP2C19 C. CYP2D6 D. CYP2E1 E. CYP3A4 Kaplan sais that CYP3A4 is inhibited by ethanol (acute) and induced by ethanol (chronic), in addition Dr Trevor in his video lectures sais that ethanol and acetaminophen get metabolized to a hepatotoxic compund by this isoform. But that was marked as an incorrect answer in USMLE 123. Any suggestions?
