hi guys !
i need urgent answer for following questions:
1)i have SSN so do i also have to right the id card no of ny country of origin?
2)another important thing is that they are asking about clinical clerkship dates which i dont have just written as 5 weeks in 4 year and 7 weeks in fifth year for medicine for example.so what should i write there?and also about physicians name as we had different for 4 and final years.
please answer my questions as soon as possible , i will really appreciate it.good luck to all.

Hi aiysha,
well yes you have to tell them your country of origin, and clerk ship is what you did after you complete your medicine, it means after 5 yrs few people did house job, its HOUSE JOB, also called clerk ship.
