A 55-year-old white male is brought to the emergency department for weight loss. He appears thin, emaciated, and unkempt. The patient demonstrates normal cognitive ability, no hepatosplenomegaly, and no fruity odor to his breath. Laboratory results reveal normal liver enzymes, insulin levels, and adrenal axis. the answer is D In prolonged fasting, liver is busy doing ketogenesis and stops using amino acids for gluconeogenesis.

funny how they can say it is D and the patient doesnt have fruity odor, I guess the patient is on the verge/just begun ketoacidosis and so gross fruity odor is not there! :roll:
