Just based on physical examination in the clinic, when a patient comes in with complaint of hand pain. DJD involves all three joints, MCP, PIP, DIP joints and also Heberden Nodes in the DIP and Boucher's node in the PIP. In RA, there is ulnar deviation of the MCP, Swan neck deformities (extension at the PIP with flexion at the DIP) and also Boutonniere deformities (Flexion at the PIP with extension at the DIP)
