when we diagnose peptic ulcer clinically, can we prescribe antibiotics for Helicobacter pylori as part of treatment OR just antisecretory drugs are enough??

Antibiotics are only prescribed If we have clue of H.Pylori infection,

agree with robin..now for revision sake..lets discuss the tests for H pylori

2. invasive

thanks. But I mean if we need to look for H. pylori? We diagnose someone as a peptic ulcer case CLINICALLY. Now what is the next step? Go for antisecretory drugs as trial OR do urea breath test and ..to find H.pylori and include antibiotics as a part of theray.

In my opinion,we should first look for H.pylori as the possible etiology as it is largely the main etiology.After ruling out Hpylori,we can go ahead with PPI.As if there is no H pylori,there is no need for antibiotics.

WE SHOULD TRY TO FIND OUT PRESENCE OF H PYLORI FIRST BY BREATH TST , UREASE TEST ,SEROLOGY ETC IDEALLY BEFORE PUTTING PT ON PEPTIC ULCER MEDICATION.

agreed..choose a nonivasive test initially..the breath test sounds good. and treat with triple regimen therapy.

But kaplan says: after diagnosing peptic ulcer clinically in someone less than 45 years old, so go for PPIs as a trial!!! I don't know wether you use other sources also? Because somethings that you sar are not mentioned in kaplan.

I´m agree with nadiabarati!!!... When you diagnose clinically PUD in a young person without prior history of taking NSAIDs and other risk factors... we suppose the patient has a H. pylori infection in a 90% BUT I think to avoid resistant to atbs we need to have the clue of infection!...ie NMS USMLE step 2 Q-book they have a question very similar but they ask you what´s the change of the patient to have H pylori infection... I think we need to use antibiotics rationaly!!!...
