In Insuline dependent diabetics you may any of these three effects leading to Morning HYPerglycemia though reason and management is different.

-dawn phenomenon occurs with an increased level of blood sugar during the early morning hours.it is not associated with hypoglycemia during the night.

the somogy effect is characterized by wide differences in the blood sugar in the morning(hyperglycemia) and during the night(hypoglycemia).it is associated with undetected episodes of hypoglycemia during the sleepsmiling face

theoretically, release of growth hormone during night causes blood glucose elevations in the early morning.at approximately 5 AM to 7 AM . GH is counter regulatory hormone that produces hyperglycemia by decreasing peripheral uptake of glucose.

the problem effect most pts with diabetes, but tends to be most severe when associated with adolcents and young adults with diabetes when growth hormone is at peak.

diagnosis..evaluate the blood sugar levels during night to differentiate the problem from somogyi effect.

pt should document the glucose level at bed time .during the night (2AM and 4AM) AND at morning fasting levels.several nights should be monitered to validate the trend.

correction..increase the insulin or adjust the night time dose.

if predawn levels r below 60mg/dl and pt is showing sighns of hypoglycemia ,then reduse the insulin dose.if 2AM and 4 AM glucose level is high then increase the insulin dose.

encourage the pt to limit the bedtime snacks to foods high in complex carbohydrate and protein,not simple carbohydrate.

Dawn phenomenon is the normal tendency of the blood glucose to rise in the early morning before breakfast. This rise in glucose, which may be due to the nocturnal spikes in growth hormone causing insulin resistance, probably is enhanced by increased hepatic gluconeogenesis secondary to the diurnal rise in serum cortisol.

Thus, both the dawn and Somogyi phenomena are characterized by morning hyperglycaemia, but the latter is due to rebound (counter-regulation) hyperglycaemia. In cases of dawn phenomenon, the patient should check blood glucose levels at 2-4 A.M. Administering intermediate insulin at bedtime can ameliorate the dawn and Somogyi phenomena.
