MCC of cor pulmonale is chronic hypoxia, causing pulmonary vasoconstriction and RVH, independent of LVH. A is the only answer that corresponds to a hypoxic state.

Asthma was what I too had selected as the answer but got it wrong. Then after I saw the answer, I recollected my days in internship at med school where my prof used to mention many times, "the commonest cause of right heart failure is left heart failure!!"
So the answer is C ! smiling face

but meg COR PULMONALE is the right heart failure due to lung pathology,is that right?

COPD 's complication is chronic corpulmonale.

hey guys...
shouldnt the answer be A...!
the choice of pulmonary embolism does look attractive...but, pulmonary embolus could be a single event or a series of recurrant emboli. only recurrent emboli will give a picture of cor pulmonale. asthma is almost always a chronic problem and surely leads to chronic hypoxemia and pulmonary hypertension and hence......
cor pulmonale!

yes you are right about that but the Q is about cor pulmonale ,means prior to heart failure there must be pulmonary disease,causing right heart failure

No, cor pulmonale is always isolated from LHF and is NEVER caused by left heart failure. The statement about MCC of RHF being LHF is correct, but by definition, cor pulmonale is primary pulmonary hypertension due to long-term hypoxia, which causes right heart failure.
