this would contain awards won in extra-curriculars while in medical school? also, positions of leadership?
or should it only have stuff like gold medal, or the various societies present in US medical schools

agreed, blank column doesnt look good. but then what are you going to put in 'other awards/accomplishments' in 'Misc' section when you have put extra-curricular stuff in 'Medical school awards'
