How can the two diseases be differentiated based on clinical presentation and basic labs??

AIDS, there is a history of AIDS around, at least a clue...e.g: immunesupresed boy whose mother was a drug abuser. and now presents with recurrent (whatever) otitis media...labs: ELISA AND WB are not very effective in a newborn, so do PCR, then confirm when he is older 1 year or so...

SCID: once you have ruled out AIDS, put your money on one of those immune deficiencies, so measure immuneglobulins.

Frank made an excellent description, I would just like to add that SCID is as the name implies very severe, presents very early on, many infections simultaneously from viral, protozoal, bacterial or fungal sources. In this disease humoral and cellular immunity is absent, key difference with AIDS.
