What is the mechanism of heparin induced thrombocytpenia?

  #2

patients recieving heparin develop antibodies to antigens on platelets and develop thrombocytopenia.

  #3

agreed. also the pt may develop thrombosis coz of the same reason.

  #4

antibodies to heparin-platelet factor 4 complex, resulting in immune mediated destruction of platelets

  #5

Good work. This appears to be a high-yield topic, so know everything about this!!

  #6

Let us push the thread ;

What is the DOC for HIT (heparin induced thrombocytopenia) and what is its MOA??

  #7

a suitable alternative anticoagulantwink

As far as I know, thefirst thing that needs to be done is stop heparin. There is do drug of choice for HIT and the only that can be cosnidered is using an alternate form of anti-coagulation, like lepirudin which work by directly inhibting thrombin.

  #9

Danaparoid can also be considerd as an alternate Rx

You are right;

Drugs which are commonly used are thrombin inhibitors which are of two types
  1. Recombinant Hirudine which is also called Lepirudine Synthetic Hirudine analogue which is Argatroban

The third drug which can work is Danaproid but it can cross react with Heparin antibodies.

Now my next question what are different types of HIT?

GL

  #11

there are two types: HIT 1 and HIT 2. do not remember the differences between the two but I believe HIT 2 is worse??

  #12

Correct GDS2008

Next question

As side effects of Heparin bleeding is common to all anticoagulants so we do not need to mention it.So

Acute use of Heparin S/E = HIT

chronic use of Heparin S/E = ???

  #13

osteoporosis?

Absolutely correct Som

Now next question

Which electrolyte disturbance can occur with Heparin use ??

  #15

Are you sure???

I jus found tht heparin is administerd in the form of heparin sodium...so hypernatremia?

Dear Som

One friendly word of caution. No doubt theoretically when you administer Na ,it should cause Hypernatremia. But practically Hypernatremia or Hyponatremia is disturbance of Water metabolism not that of Na (of course exceptions are there). But in case of Heparin Na is not disturbed.Let us wait for others to reply.GL

  #19

bleeding, osteoporosis, elevated transaminase, hypokalemia (decreases aldosterone production). However, non is common except bleeding

  #20

and a very good point sea_gull reg hypernatremia................
