A 55-year old man with hypertension comes to the emergency dept 6 hrs after the acute onset of epigastric pain, nausea, and vomiting. He takes spironolactone and a Ca-channel blocker. His temp is 101, BP 140/80, HR=125, RR=22. Abdominal exam shows epigastric tenderness and guarding; there are no masses. Bowel sounds are decreased. Serum studies show Ca of 7.0, amylase of 7600, lipase of 5650. Which of the following is the most likely cause of his symptoms?
