u can start by saying that "I'm sorry to hear that.I understand that it's a hard time for u but if it's ok then I would like to ask u some questions & then examine you to find out that u r doing ok...alright?......."plz be assured that everything we discuss is confidential.So feel free to tell me everything...(smile)"...... 'can u plz tell me more about it like when & where did it happen?"...when she explains then ...."did u get hurt?".."any pain or body aches?"...."have u noticed any rash or growth in genital areas?"..."vaginal discharge?"......"were u on any contraceptives at that time?"...."When was ur LMP?""any spotting?",...."pain during sex?""have u discussed it with any1 else?""ur partner knows about it?"..."tested for HIV?"..."Last pap smear?"...& Then start taking depression Hx too...like"how's ur mood?,sleep disturbances,hopes for future,appetite,energy level.conc.,libido,interests & hobbies,anxiety,suicidal thoughts & plans...etc....& then the rest of PAM HUGS FOSS....besides the ones that u've already asked.

3. I am the doctor on duty today and here to ask you a few questions. (The information is not confident because the history will be turned over to the CID, Crime Investigation Division, Sex Crime Unit, and to the State and also used in the court to both DA and attorneys) Both DA and attorneys can request for medical records ! In US, all rape cases record belongs to the 'State" !! Please correct this !
4. Can you tell me when the incident occurred and what were you doing at the time.
(I was walking towards my apartment at 1 a.m. Saturday morning and a car stopped and a man jumped out and pushed me on the ground and dragged me to the back of the dumpster behind the apartment)

(He then started to pin me down on the ground and I bite his right forearm and yelled. He covered my mouth and pulled a knife out and cut my button and then the blade of the knife was pressing my neck. I was very scared)

(He torn my top off and pin my shoulder down with his arm and used force with his right hand to spread my leg and pulled my pants down.)

(Yes, I did ! My head was hit very hard several times and the grass got into my eyes and my right inner thigh was bruised.)

(No)

(crying, I can't remembered)
11. That's O.K. and did you have any pain now?
(I have a lot of pain in my leg and thigh when I was pinned down. He proceeded to pull my pants down and torn my underwear and we struggled for a while and I almosts passed out. I cried and asked him to stop but he managed to penetrate inside me and kept on for a while until I saw the headlight of a car pulled to the back of the apartment and the man jumped up and pulled his pant and ran)

(No)
13. Did you call the police right the way? Or did you go home to take a shower?
(I was crying and someone came and helped me up and I managed to get back to my apartment and took a shower. I felt dirty.)
14. Did you change any clothings?
(Yes, I did put the torn clothings in the bag and changed into a T-shirt)
15. I will ask a nurse to hep me to do a physical examination and run some tests and do a pelvis examination and comb your hair and collect some specimens. There will be a social worker here lator on to help you anything. Thank you very much.
