PPD Test:
What is considered positive in:
1. An immunocompetant person
2. A sick person with primary depression of their immune system
3. An immunocompromised person (e.g., organ transplant, HIV)

(how many mm induration?)

1. >15mm
2. >10mm
3. >5mm

1. >15mm, plus no risk. if immunocompetent and risk (homeless, developing nations, IVDU, etc..), go for >10mm

2. this one counts as immunocompromised (but depends on histiry) so >5mm

3. >5mm (but a just transplanted can present with anergy, for obvious reasons)

*anergy=immunosuppression, malnutrition, old age, AND does not rule out TB.

I think person with immunocompromise is >10

if you say >5 you imply:
HIV +ve
close contacts of active TB
steroid use and organ transplant
with abnormal CXR with old ,healed TB

I would agree with frank100 here. I am not exactly sure what is meant by primary depression of the immune system (I would imagine immunodeficiency syndromes).In that case, I will be inclined to choose >5. However, please correct me if I am wrong. This is an important concept for the exam and we need to be clear on this. Thanks

5mm in any defect of cell mediated immunity or other form of immunosppression. (Washington Manual 34th ed)
