a manometry is regarded as the diagnostic study whilst esophagoscopy is reqd to r out maligancy. m y question is that do we need a biopsy for confirmation or not? once we have the manometry results can the pt be treatred accordingly? also y isnt a biopsy showing a lack of ganglionic neurons THE diagnostic test ( and the only way to get it wud be a esophagoscopy)?

i dont think we need a biopsy to diagnose achalasia. THE test is manometry. at esophagoscopy which is done to rule out cancer (cos achalasia is premalig), a biopsy can be taken and at same sitting, the dilatation is carried out. i will look it up more. http://72.14.203.104/search?q=cache:CmItm7cdVxYJ:...

http://www.cag-acg.org/guidelines/Pdf/cockeram.pd... this struck me as i tot LES was the main problem dx of achalsia.. REQUIRED Features --Esophageal body –lack of peristalsis sometimes present Incomplete LES relaxation Elevated LES pressure (>45 mmHg) Elevated intraesophageal pressure
