A 28-year-old woman develops temperatures to 39.4 C (103 F) and substernal chest pain 3 days after being treated by her dentist for gingivitis. An x-ray film of the chest shows an enlarged cardiac silhouette. On pericardiocentesis, 250 mL of cloudy fluid containing large numbers of neutrophils is removed. Which of the following is most likely on examination of the fluid?

A ) Acid-fast bacilli

B ) Budding yeast forms

C ) DNA virus

D ) Gram-negative bacilli

E ) Gram-positive cocci

INFECTIVE ENDORCADITIS



IE may be acute or subacute; org assoc with:

- Acute IE are generally more virulent (m.c. S. aureus) and are capable of infecting previous normal or abnormal valves.

- Subacute IE are usually less virulent (m.c. S. viridans) and infect previous damaged valves

Overall S. viridans is the m.c. pathogen causing IE followed by S. aureus.

In pts with prosthetic heart valves, Coagulase negative Staphylococci are m.c.

In IV drug abusers S. Aureus is the m.c.org isolated in the blood.

Mitral valve most common involved. However in IV drug abusers the Tricuspide and aortic valves are equally affected. predisposing to embolization (strokes, splenic infarcts)

But this is pericarditis and not IE....or is pericarditis a sequele of IE ??

Straight out of First Aid:

"Complications of Bacterial Endocarditis include Chordae Rupture, GN, Emboli, and SUPPURATIVE PERICARDITIS"

Dental work typically causes SBE via Streptococcus viridans group, of which there are many subspecies.
