I am ok starting residency with both J1 and H1, however most of the top programs take only J1 so while filling CAF if i fill in both J1 and H1 in the 'expected visa' section, is it going to be detrimental for the j1 only hospitals? or should I stick to just J1?

however mine is for surgery. medicine h1 is not that difficult

If you would seriously consider both visas, then check both.
