Three weeks after being bitten by a friend's puppy in Mexico, a 32-year-old man has tingling at the site of the bite followed by severe agitation and disorientation. The initial wound healed without complications. The puppy died 2 weeks ago. The route by which the infecting virus entered the brain is? A) blood-borne virions infecting cerebral capillary endothelial cells B) blood-borne virions infecting choroid plexus cells C) blood cells that cross the blood-brain barrier D) transport within the axons into the central nervous system E) virions carried by lymphatic drainage to the brain

Yes it's D Just wanted to mention that I took NBME form 1 a couple of days ago and this q wasn't in my test for sure, although most of the other ones posted were...So I guess there is a slight difference depending on when you take the test, right?

D. When a human is bitten, the virus (rhabdoviridae)replicates locally at the wound site for a few days, then migrates up nerve axons to the central nervous system, causing a fatal encephalitis.

So, let me get this straight - you guys already have the questions for part of your test and you just memorize answers????? I'm an MT, and we don't get this luxury. We're FAR down the pecking order from MD's and you guys get the big bucks :-)....
