Ok nadiabarati, this is my understanding: 1. No matter if u place BCG or not, if PPD over 10mm give INH (for these increased risk patients, 15mm for no risk normal people). 2. U place BCG to prevent most fatal cases of TB, not to prevent TB itself. So the anwser to what u ask, is place PPD and if it is positive get CXR (to rule out active TB) and start INH. If PPD neg and CXR neg why start INH?

BCG is only useful in preventing maningitis and to some extent miliary TB in kids...it has no significant role in preventing the overall disease. its mainly used for Dx
