63 yr old man comes with h/o lethargy. examination is unremarkable except for a BP of 192/ 112
routine blood tests reveal:

k+ 2.4 mmol/l

HCO3 34 mmol/l

what is his most likely diagnosis

a. diabetes mellitus
b. cushings
c. pheochromocytoma
d. conns syndrome
e. bartters syndrome
plz with explanations...

Does cushing causes hypokalaemia and alkalosis?
I think the only one here that causes both sign is bartters syndrome.
Bartters syndrome:
A clinical syndrome characterised by enlargement of certain kidney cells, alkalosis, hypokalaemia and increased production of the hormone aldosterone. Common in children and may be seen in dwarfism.

D) Conn's.
Hypokalemia,alkalosis,high BP----Conn's. Bartter too has hypokalemia and alkalosis,but the BP is normal. Cushing can have hypokalemia and alkalosis,esp with ACTH secreting tumour,but since they asked more likely,maybe it is Conn's. What is the answer?

I think it's Conn- hyperaldosteronism with hypokaliemia, hypertension and increased HCO3; I am not sure about the mechanism for alkalosis : is the increasing urinary secretion of H which means more NH4 in the lumen and more HCO3 wich is reabsorbed?
Barter disease- Hypertrophy /hyperplasia of the juxtaglomerular cells, producing hypokalemic alkalosis and hyperaldosteronism, characterized by absence of hypertension in the presence of markedly increased plasma renin concentrations, and by insensitivity to the pressor effects of angiotensin; usually affects children

both of them have similar caracteristics,

CUSHING .- increase BP, and hipokalemic alkalosis

CONN .- hiperaldosteronis primary , increase BP,hipokalemic , and alkalosis.


but the question said the patient not have another caract in the exam , so it could be CONN
