Tensilon is NOT the best initial one.

anticholineesterase antibodies is NOT the most accurate

neostigmine is not the answer here.

Initial test - Acetylcholine receptor antibody
Accurate test- Electromyography shows a decremental decrease in muscle fibre contraction on repetitive nerve stimulation.
Initial treatment - Steroids --- if no effect Steroids + Azothiopurine
in myasthenic crisis - Plasmaphoresis and intravenous Ig
Thymectomy - in postpubertal and those less then 55 with generalized myasthenia before immunosuppresive therapy
Pyridostigmine for symptomatic treatment

all i knew was...
anti ach rc ab gives less false positives. tensilon may be positive eg in ALS
EMG-->decremental on repetitive stimulation
neostigmine and pyrido are SYMPTOMATIC. steroids and azathioprine are curative. of these steroids have an onset of axn within approx a month.
in myasthenic crisis - Plasmaphoresis and intravenous Ig
the latter is supposed to be easier to arrange but both are equal in efficacy. the cant ask u to choose between them.
