Question 1: I have completed 3 years of training in UK and I would like to mention this in my CAF as residency because that is basically what it is!! but... in UK you rotate in different hospitals every 6 months and they all have their own directors and supervisors. when listing all as seperate residency it looks like I have done 5 residencies all 6 months long!!! looks a bit awkward to me.... please advice will that be OK??? Question 2: when writing the name of my supervisors and directors... what is the usual sequence?? Dr. John Smith or John Smith,MD or Smith J,MD or Dr.J.Smith??? in case of a prof. John Smith, professor of.... or Professor John Smith or Dr.John Smith, Professor of... Please help me with these... I very much appreciate your help many thanks

I have put them as seperate 6 months job.. My supervisors titles are Mr, Miss and also military title like Lt. Col. ...... so I have put them as Miss Cilla Reid etc.. no titles as they are on LORs anyways. ... I suppose u can explain in job description if ur job is a part of rotation. Like SHO Orthopaedics part of Emergency Medicine Rotation or Surgical rotation... I hope tjis helps.. Atleast this is what I have done so far.... I am open for suggestions though..

thanks for answer... problem is: there is no place in CAF for description of your residency.... have you put it under experience???

CAFs are filled when I see them, so I can't help you here.

I have put it in section of previous training where it states : Page 4 - Previous training : Please include each residency, fellowship, and/or osteopathic internship you have completed or are currently in. Complete the required fields and Save

on page 5 - Work experience where it says : (Include Clinical and Teaching experience as work experiences, include all unpaid extra-curricular activities and committees you have served on as volunteer experiences. I have put my voluntary works, teaching experience + research, clinical attachments in UK. At least this is waht I understand from the form.
