A 55-year-old woman who is obese comes to the
physician because of daytime drowsiness, early morning
headaches, and swollen ankles. During the interview,
she says, "My husband tells me I snore too loudly. He
can't get any sleep because of it." Echocardiography
shows an enlarged right ventricle and poor right
ventricular function. Which of the following is most
likely responsible for the development of right
ventricular failure in this patient?

A. Increased pulmonary artery pressure
B. Increased right ventricular preload
C. Pulmonary embolism
D. Pulmonary valve stenosis
E. Transient hypoxia in right ventricular myocardium
