chest xrays are not routine tests. so the gen populace is not screened using it. ( we should let the embassies know this. they always screen with a CXR )
it may be done if PPD is positive.

DONT think of Vit b12 diet in a vegetarian of one year...takes atleast 3-4 years..think more of pernicious anemia ..

DONT think seminoma in the young adult with suspected testicular tumor with HIGH AFP. its never produced in a seminoma. ( emryonal ca more likely)

never use topical steroids to reduce inflammation when thinking of viral keratitis. this will act like a virus fertilizer.

yes duplex scanning is important but vascular surgery = Prevention, not treatment

NEVER do MRI to look for intraocular foreign body. it is containdicated ( in case object affected by magnetic field of MRI)
the sequence is gross exam---> fluorescein/slit lamp-->ultrasound or CT if high suspicion

Never give aminoglycosides in infant botulism ( Reason:as aminoglycosides kill the bacteria and subsequent release of toxin can worsen the condition).
Never give Rubella vaccine to a pregnant woman. ( However, if a pregnant woman had an inadvertent rubella vaccine during her pregnancy---Mx: Reassurance and this is not an indication for abortion)
In occult bacteremia in children age 3mths-3 yrs: No need to repeat the antibiotics if the child is non-toxic and has a normal physical exam, even if the blood culture is positive.
Don't use doxycycline/tetracycline in children < 8 yrs & pregnant women. ( exception: In rocky mountain spotted fever, doxycycline can be used even in children < 8 yrs, as it used for short courses)
Never give blood transfusion/blood products to IgA deficiency pts. without proper precautions----like proper screening and irradiation of blood products, in case of a real need for transfusion in such pts.) Reason: high risk of anaphylactic reactions.
Never use Aspirin in children ( exception: kawasaki's disease and juvenile rheumatoid arthritis). Reason: risk of reye's syndrome.

Never repeat rectal exams in acute bacterial prostatitis. Even the first (the only one time P/R exam ) rectal exam should be done gently with care, as vigorous manipulation may lead to septic shock.

never give steroids in Guillan Barre Syndrome it makes the condition worse.
