A 54 yr olde female has reporting to your office with complain of incresing voiding, dry mouth and profuse sweating at night. her blood glucose level is 248 mg/dl. the patient present with the following lipid profile too..

HDL...27 MG/DL

LDL...185 MG/DL

TG...495 MG/DL

what theurapetic agent would give an increase in HDL and decreases in TG?

1..glyburide

2..repaglinide

3...pioglitazone

4...acarbose

5..insulin

6...metformin

7...niacin

8..gemfibrogil

9..lovastatin

8 gemfibrozil(decreases TG, increases HDL a bit and also potentiates hypoglycemics, so good in this pt)

both gemfibrozil & niacin cause increase in HDL & decrease in TG. wats the ans kaurji.

One of the side effects of Niacin is hyperglycemia. It isn't the drug choice for a patient with DM. 8 Gemfibrozil

symptoms suggestive of diabetes mellitus........and there is lipid profile is also abnormal....so we have to manage both......

answer is pioglitazone......

drpkaur, whats the moa of pioglitazone. i checked in kaplan but the drug is not mentioned.

pioglitazone belongs to thiazolidenediones-mechanism of action of these drugs is that it stimulates PPAR-gamma receptors-the nuclear receptor regulates transcription of genes encoding proteins involved in CHO and fat metabolism
