Regarding the question below..I read that asymptomatic adenxal masses have 0% chance of malignancy if post-menopausal. I also read that if its endometriosis there will be pain to accompany the adnexal mass. That being said, I still don't know the answer:

An asymptomatic 65-year-old woman comes to the gynecologist for a routine examination. Physical examination shows a left adnexal enlargement that was not present 1 year ago. Which of the following is the most likely diagnosis?

A) endometriosis
B) leiomyoma
C) ovarian tumor
D) polycystic ovarian disease
E) ruptured luteal cyst

hi...can u explain why its that and not the other choices. obviously its not endometriosis cuz that would involve pain..and its asymptomatic so no malignancy should be involved.

this is likely to be ovarian tumor the others can easily be ruled out

not likely to endometriosis because she is postmenopausal and pain is caused in endometriosis due to stimulation of the ectopic tissue eith estrogen

leiomyoma is ore common in black women of reproductive age -- they also thrive on estrogen and this woman is postmenopausal
leiomyomas are not usually adnexal masses -- usually in the uterus

PCOS --more in women of reproductive age ---should be bilateral in most cases

amitdoc where did you read Adnexal mass has 0% chance of malignancy postmeno. ? in fact next to endometrial carcinoma it is the 2nd most common GYN tumor !

The prevalence of ovarian cancer is low in young women but increases with age, with the highest rate occurring in the eighth decade of life. !!!

and talking about assymptomatic :
It is difficult to detect ovarian cancer early – only about 25% of the cancers are found in the easily treatable stage, before they have spread beyond the ovary. Because the symptoms of ovarian cancer are subtle and non-specific and since there are many non-cancerous conditions that can also cause similar symptoms.

well, any post menopausal is a tumor untill proven otherwise.

on the other hand, choices A,B, D, & E are hormonal dependant, & since she doesn't have any, it has to be C

endometrial 55 years
ovarian 65 years smiling face
