there will be many questions in the exam that ask about next step in diagnosis, How we can choose among doppler, duplex, USG, CT with contrast or without, MRI...Can anyone help to specify the uses and indications of each of these??

there is no hard and fast rule for them. if you have no clue about what the next step shoudl be, then take a guess for the most non-invasive and cheapest option. otherwise, as you study along, put your heart to the different options and learn them.

generally, doppler, duplex are to investigate vascular integrity. duplex is more fanciful than doppler.

USS will generally help to differentiate and localise solid and cystic lesions. no use in the brain (except in children b4 fissures fuse) and bone. also of no use in retroperitoneal structures cos bowel gas will overshadow. of course, inevitable in obstetrics.
CT plain is fine for bony lesions. contrast is used to delineate soft tissues better. may be oral or iv contrast depending on the tissue.
MRI is presently the best modality for soft tissue investigation. is better than CT for soft tissue.
radionuclide scans for bone mets, thyroid, infections and hemorrhages.

so based on the question stem, you can hazard a guess as to which is more likely if you have no clue about the condition.

.am trying to compile some of the 'knotty' indications as i go along. please visit the thread http://www.prep4usmle.com/forum/thread/38920 and add your input. gradually, we will build our database.
