an empyema + pus in pleural cavity (an abscess, remember. involves the parenchyma) an empyema can be the result of a parapneumonic process. however, chest trauma for eg can give rise to it too.

this is what i think An empyema is treated with prompt chest tube drainage with the use of parenteral antibiotics. so the difference in managment wud be the empiric antibiotics. for the parapneumonic effusion, ud cover the suspected organisms. for an empyema due to trauma, u'd cover skin flora.
