in mechanical intestinal obstruction due to adhesions, kaplan says: observation. Why not surgery?

strangulation=gangrene??

incarceration=obstruction???

because adhesions are due to previous surgery in most cases and sometimes follow septic conditions. surgery would break up the present troublesome adhesions but would lead to formation of more adhesions. the other reason for the 'suck and drip' approach to this kind of intestinal obstruction is the possiblitiy of forming fistula between loops of bowel when the adhesions are broken by surgery.

surgery is therefore the last resort in cases due to adhesions.

strangulation means when the blood supply of a segment of bowel is cut off so it can lead to gangrene.

do not have a good definition for incarceration.

it's observation because most of the cases of intest obstruction due tu adhesions are self resolved, you only need to give IV fluids, observe, serial physical examinations. i think the limit is 2-3 days, if it's not resolved in that time, then you have to operate.

Dear Nadia
I know that you scored pretty good in step I.But from your posting here and other forums ,it seems that your basics are not good.I cannot comprehend it. Why is it so??

I don't know either! Maybe you should call the exam givers and tell them your finding and ask them to modify my score. How about you? what was your score in step 1?

I dont know any of you but it seems to me that the whole point of this forum is to explore our weaknesses and learn from our mistakes. Scoring 99 or scoring 71 does not makes us "more" than anyone else. It just reflects how prepared we are for THAT particular test. I was never the best in my class but I scored high because I was well prepared. I wish the both of you the best grade possible.
