6 days after undergoing a laparoscopic cholecystectomy for acute dholecystitis, a 35 yr old woman comes to the physician because of feaver and abdominal pain for 3 days . She is jaundiced. Her temperture is 38 oC abdominal examination shows distention ans incisions that are healing normallly. Leukocyte count is 12,000/mm3, and total serum bilirubin level is 7.9 mg/dL. Which of the following is the most likely cause of the jaundice?

A. Anesthetis-related hepatitis
B.Common bile duct injury
C. Fulminant hepatic failure
D. Reaction to perioperative antibiotics
E. Subhepatic abscess

I would go with B

I am not sure. I picked E, thinking it is an infection...

yes, (e) is a better guess, but I join the "clueless" club.

B , becuase of serum bilirubin level (7.9 mg/dL)

i'd go with E but i am not sure either. just making a guess because of fever, abd distension and maybe jaundice is caused by the compression of bile duct by abscess.

i th ink answer is B

I iam feeling I have go thru this answer before is it the same question as uw

B is the answer, as common bile duct injury is the most common postop complication. Reason : normal anatomic variants of the biliary system are damaged and lead to chemical peritonitis. Abcess would not present on 3rd post op day but later on.
