A previously healthy 32 yr old woman comes to the physician because of a 4 month history of depressed mood and progressive difficulty with her work as an accountant because of diffidulty concentrating. She takes no medications. She was adopted, and her biologic family history is unknown. Physician examination shows occasional facial grimacing and twitching of the roght hand that she tries to disguise by smoothing her hair. Mental status examination shows a depressed mood and mild psychomotoe retardation. She s able to recall two of three objects after 5 mins and perform serial sevens slowly with one mistake. An MRI of the brain shows atrophy in the heard of the caudate nucleus. Which of the following is the most likely diagnosis?
